In a sex-change surgery..where on earth does the "extra" stuff come from??

Sorry i was wondering because i be watching larry king live and they had culture awaiting sex changes..and im close to ok..then i guess i be thinking to deep into it..and i thought where on earth does the extra skin come from (in a female to male)..next like where on earth the stuff go to within male to womanly?..i dont kno just the chance thoughts in my person in charge.

OK.there are a few different procedures that may be used, but I'll stick next to the commonest ones.

In vaginoplasty for transsexual women, the skin of the penis is dissected off, the erectile tissue removed, and the neural and vascular supply to the glans (head of the penis) discreetly dissected out; The testicles and spermatic cords are removed completely. A cavity is prepared between the rectum and the bladder, and the penile skin inverted to line it; this forms the vaginal conduit. the glans is trimmed down to create the clitoris, complete with the nerves and blood supply that be dissected out earlier. The skin of the scrotum is used to create labia. If the surgeon is competent, and in attendance are no major post-op complications, the vagina thus created is visually and functionally indistinguishable from an XX vagina, once heal.

For transsexual men, the surgery is more complicated, and despite recent advances, still not completely tolerable, so many transmen choose not to own phalloplasty at all, although all will have hysterectomy and mastectomy.
For those that do longing to have a penis constructed, in attendance are two main option;
If testosterone therapy have caused the clitoris to develop sufficiently into a micropenis, a procedure call metoidoplasty can be performed, whereby the ligaments supporting the micropenis are cut. this allows the penis to droop lower, and at a more natural angle. This may do to produce an acceptable penis, which logically is erectile.
If the penile development is insufficient for metoidoplasty, a penis can be constructed from a flap of skin and subcutaneous tissue taken from the tummy, thigh or even the forearm; this will contain no erectile tissue, so an artificial silicon pump (as is used for XY men with erectile dysfunction) can be inserted to produce erection. This form of phalloplasty occasionally produces an anatomically accurate penis, but it can be made an appropriate size and shape.
In either bag, prosthetic testicles can be inserted into a scrotum created from the labial tissue;and the urethra (the tube that you urinate through) needs to be created contained by order to engineer urination through the penis possible, otherwise the transman will still have to sit down to pee. This is a difficult procedure, and recurrently not completely successful.
