Sounds particularly much like an urinary tract/bladder infection and possibly an out of the ordinary connection between the bladder and the intestine (vesicoenteric fistula) which allows nouns to enter the bladder and sometimes enabling germs that produce gas to enter and grow in the bladder. Air bubbles can appear contained by the urine (pneumaturia)
If you have have divertivculitis it is possible to develop a fistula due to the bowel being so inflammed specifically causes tunneling into neighbouring structures. This might explain some of the perineal pain (between your bottom and testicles)
