I'm 16 years old(male) and after I poo sometimes I see blood on the toilet paper. It comes and goes(usually a couple weeks I'm fine and it impulsively happens again).

If it is tough to push out the bowel movement, or you are putting off going until the stool harden, it can cause little mini-tears to your rectum on its course out. Treatment for this is plenty of fiber (fruits, vegetables, whole small piece cereals, etc), lots of water/fluids, and perchance even a stool softener you can buy at the drug store, such as colace. It is not a problem to ignore. Continued straining can mete out you many problems down the road, so if your poo is complex and round or difficult to pass, time to bear action to correct it.Fresh blood is usually from difficult to pass by stools or hemorrhoids.
Black, tarry stools mean bleeding further up in the digestive tract.
Increase fiber, increase fluids, and if that doesn't work, consider a stool softener. See your doctor if it continues.

It's likely lately large or rock-hard poo causing it. Eating a diet high in fiber and lower contained by processed foods will help.
